I hereby acknowledge this release from liability for accidental injury or illness which I may incur as a result of participating in any physical activity. I hereby assume all risks connected therewith and consent to participate in this program. I agree to disclose my physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in this program.

I agree to participate in the National Safe Boating Council’s training course, run by Cool Breeze Charters LLC, or a portion thereof. I understand that this program includes both classroom and on-water sessions, and that water-based activities inherently carry risks, including immersion and death. I acknowledge that participation may involve serious injury, arising not only from my actions or negligence but also from others’ actions or environmental conditions. I agree to follow all rules set forth by the National Safe Boating Council (NSBC) and Cool Breeze Charters LLC. I will obey the instructor(s) and supervisor(s) and wear all required safety equipment, such as personal flotation devices, non-slip footwear, and weather-appropriate clothing. Prior to departure from the ramp or dock, a pre-departure checklist and safety briefing will be conducted. If any equipment is deemed unsafe, I will immediately inform my instructor or supervisor and refrain from participation until the issue is resolved. I certify that I am in good health and able to participate without adverse effects. I will notify the supervisor and instructor(s) of any health conditions or medications that may impact my participation. Having read this waiver, I hereby release, waive, hold harmless, and discharge the NSBC, NYBoatLessons.com instructors, sponsors, representatives, volunteers, government organizations, and any affiliated boards, trustees, employees, or volunteers, as well as the owners and lessors of premises used for the event, from any claims, demands, losses, or damages to person or property, caused or alleged to be caused in whole or part by negligence or otherwise.
